A lot of NPCs have [ ! ] over their heads after the patch

Not sure if I'm the only one having this issue but a lot of NPCs appear as-if they have knowledge to give me but they don't because I already have it. It has happened before months ago. The NPC will have the [ ! ] (yellow exclamation mark) over their head.
Is anyone else having this issue? (PC/WEB client)

This happens alot since PA took over. With every patch the yellow marked NPCs change for me. But most of the time it's Fughar and the balloon NPCs.

It's not for the first time. I remember this happened before too. Several times at that.

happens all the time to me, fughar most of time and after they add new npc that is in every city they also get this from time to time, that alchemy shai npc and imp cooking shai npc for example

It happens all the time, mostly after patches. Very annoying.
