Garmoth's Heart is a rare drop that can be fused into certain main weapons, off-hands, and Awakening weapons to grant additional effects.
To fully enhance your power, you will need at least three of these items, one for each weapon slot.

How to Obtain
There are several ways to obtain this item, but most of them are challenging and require a significant time investment.
◈ World Boss Garmoth
There is a small chance to obtain Garmoth's Heart as a drop from the World Boss Garmoth. While Garmoth appears multiple times each week, you can only receive loot rewards from her up to three times per week.

How to Claim Garmoth's Loot
1. When entering Garmoth's Nest, a special quest will automatically appear next to your minimap.
2. The quest is completed after defeating Garmorth and obtaining a special loot item. (Click the chest icon in the middle of your screen.)
3. The quest will grant you the item Garmoth Reward Bundle. Opening this item has a chance to grant Garmoth's Heart or Garmoth's Bloodstone.
* Keep in mind that it is not guaranteed that you will obtain these items, and their drop chance is really low.

◈ Garmoth's Bloodstone
This item has a low chance to be obtained after opening Garmoth Reward Bundle. Once you have collected 10 Garmoth's Bloodstones, you can exchange them for a guaranteed Garmoth's Heart. To do this, accept the quest "Happy Crimson Accident" from Kukao at Garmoth's Nest.
◈ Garmoth's Heart Questline
There is a questlines that allows you to obtain a guaranteed Garmoth's Heart. However, this quests is not easy to complete and it will require you to obtain different rare items. You can start this questline at the NPC Black Eyeroot in the Crypt of Resting Thoughts. Keep in mind that in order to accept this quest, you will need first to complete O'dyllita main story line.

Inverted Heart of Garmoth
In order to use Garmoth's Heart and infuse it in your weapons, you will have to modify this item and transform it into Inverted Heart of Garmoth. Head to Hughol, the blacksmith in Duvencrune, and accept the quest "[Weekly] Inverting the Energy".To complete the quest, you will need the following materials:

As a quest reward, you will receive Inverted Energy. Open the Processing window (L) and use Heating to combine the Inverted Energy with your Garmoth's Heart.
How to Reform Weapons with Inverted Heart of Garmoth
Talk with the Black Spirti (,) and click the "Upgrade/Reforge" option. Then select a weapon and Inverted Heart of Garmoth, and finally click "Reform".
How to Remove Inverted Heart of Garmoth
In order to remove the Inverted Heart of Farmoth and infuse it into another weapon, visit a blacksmith in any town or city, and purchase a Mirror of Equilibrium for 20,000,000 Silver.
Then talk again with the blacksmith and select the option "Extract", and "Extract Reform Stone". Select the weapon that has the Inverted Heart of Garmoth infused, and click on "Extract".
You will recover your weapon and the Inverted Heart of Garmoth in your inventory.
[Heidel Ball 2025 Rewards] Free Inverted Heart of Garmoth
Due to the special event Heidel Ball 2025, you can obtain a free Inverted Heart of Garmoth until November 27, 2025 (before maintenance). Simply open the Challenge Window (Y) after completing the following requirements:1. Reach 200 hours of Family playtime.
2. Have the [Knowledge] Wuju.
* If any character in your Family has already finished the "[The Magnus] Wuju's Gift...?" quest, you already have this knowledge. You can double-check via the Knowledge (H) - Find Knowledge feature.
