Theres been so many posts like this over the years but so far all thats been figured out is to make small tweaks for minimal improvements much like the guide you already tried. The issue is probably so deep into the engine of BDO that it can never resonably be fixed completely.
One major source of performance issues is actually tied to the UI so if you unlike me have any energy left for testing stuff out you could always try and disable certain elements of it and see if you find any UI part thats worth hiding when you wanna grind. Hiding the entire UI with ctrl+U makes the game run significantly smoother but its also not practical for actual gameplay.
Oh, and also be weary of were you test stuff out because there are certain zones across the BDO world were you always get sturtter/fps drop when you walk over a "seam" in the terrain. One such place I know from the past is south of Behr close to the popular herb gathering spot. Can run the game on minecraft settings and you will still get fps at that specific "seam".

My only idea at this point is that my old 4th gen CPU (6 year old) and memory connected to it are holding my GPU down and cosing in game stutters. I know that game itself has some performance issues, but I know that for some people smooth gameplay is achievable.

I would not upgrade my PC for the sake of BDO only. Seems like a costly gamble. If you have a few other modern games that you wanna play that requires greater peformance then sure.
I have also heard about ppl playing the game smoothly but I have never seen the game run smoothly on any PC ive owned or any PC any of my friends have owned so I tend to see those ppl as the "the human eye can only see 24 fps" type of players :)
Game is getting older though so every new gen of PC parts have a better chance at brute forcing a better playing experience I guess.

Stuttering continues to be a problem, it doesn't matter what resolution or graphics quality, it stutters all the same. My max FPS is around 70-90fps on max settings @ 2k resolution. I bring it down to remastered mode where it ranges between 1fps - 144fps. It just depends on the moment. The stuttering is a real problem especially in Olvia servers where it seems to be the most busy.
No issues in other games such as FF7R, Cyberpunk or CoD. No issues in other MMOs such as ESO, FF14. No issues in other modded games such as Skyrim and Fallout 4. No issues with drivers or system files.
Only BDO is a stuttering mess.
Particle effects are the biggest part of the problem, probably bad coded, just looking at the chimney in velia, the smoke coming out, i lose 10 fps, they just add more and more particle effects whit new patches, horses with effects, mobs, black spirits etc. Even with a completely new pc with i5 12400 etc. it struggle to manage them, for ex elvia ruthums sometime my fps drop from 144 to 100, is super annoying cause all that stupid effects.
Hi guys, I got two high-end gaming machines with the latest 12-gen cpu:s and nvidia 3070 ti graphic cards and I have stuttering on both pc while playing.
Last week I finally discovered the reason and it is caused by the UI. If you disable the UI using "ctrl + u" the stuttering will stop, but of course you need the UI while playing the game. So you must change the UI map from 2D to 3D and it will greatly improve your experience by having less stuttering. Devs hasn't fixed the stuttering caused by the 2D map. Also remove things in the UI you don't need, like tooltips on the button left of your screen.
Keep in mind that stuttering can also be caused by not having good enough computer parts to handle the game, but the tip above is towards players with strong pc.

I've noticed that too, it's ridiculous how some random and simple graphic effects or elements reduce the performance greatly like they were high poly or volumetric effects. I didn't notice the chimney performance drop, but try to put the camera inside or near a horse tail, the fps drops like the horse were using Nvidia hairworks or something like that and no, the tails are just a bunch accumulated flat polygons with textures lol.
Other example are the flooded areas, Titium Valley for example, when you use AoEs or throw the shai florang the spalsh effect of the water cause a momentary but big fps drop and the effect is not particularly amazing or cool. Zooming out the camera reduce problems like this a lot, but I don't like to see my characters that small like I were playing a Diablo-like ARPG or Lost Ark.
I think the root of the problems is they're are using a bad engine, developed by themselves just to run this game (and most likely in a hurry) resulting in a poorly optimized mess.
My husband set this Windows 10 desktop up to "handle Black Desert" the best he could. Everyone seems to be looking at the game client and their hardware, but how do you have your OS configured?
1. Debloated using a user made script; removing a lot of unused programs. That is to say, I won't miss Cortana or Analytics.
2. Registry edited to automatically set BDO client to run at "High Priorty" on the CPU. Dedicating the CPU resources to the game.
3. Nvidia drivers up to date and the Control Panel set to override the application settings where needed.
Of course #3 isn't going to be a factor unless you are running Nvidia. I don't know if you can work with Windows 11 on any of these features. I am not certain what else he fiddled with, but my experience is fine by my standards.

I dont understand how can someone mess so much on how pc works to get more fps, LOL
intall 3rd part programs, mess whit CPU frequency, mess whit windows registry, mess whit how cpu handle the game ecc
