That "casuals don't get to clear the highest tier PvE content" depends largely on the MMO you're playing and how said MMO is designed. Speaking from personal experience here: I played several korean MMOs, especially Aion and Blade & Soul. I also played Guild Wars 2. I dare say that games can reach a proper balance between catering to the casual audience and the hardcore audience at the same time. Aion and Blade & Soul did so until these games got infested by P2W. A few hours per day enabled you to play in the upper 10% of the playerbase - including "highest tier PvE content". The only requirement was a decent skill-level and decent knowledge of the game and its mechanics with which you could compensate for gear-differences - a dependable static also helps. Sure, there were still gaps in PvP, but PvE in MMOs is mostly a trivial and almost purely mechanical matter. Especially korean MMOs are known for their lackluster PvE content anyway. As I've said though: That's only possible in decently designed MMOs. As soon as greed hits and P2W ruins the games by introducing excessively grindy bullshit and exponential progression-systems, said games inevitable go down the drain. I only ever had problems in Guild Wars 2 due to its rather un- - if not - antisocial nature and too casual-friendly vanilla-content.

What I witness almost every day in the MMO community is the heavily twisted perception the community developed due to the community being starved of decent MMOs and good game design. Every single popular MMO on the market is dirt-old - BDO somewhat included. Lazy Peon described the current situation quite well is this video:

Every single MMO on the market is also at most a 5/10 if you were to rate their quality - including popular games like World of Warcraft, Final Fantasy XIV, Elder Scrolls Online and Guild Wars 2. If you were to rate BDO, you'd probably end up at a 2/10 since BDO barely qualifies as a framework. All BDO realistically has are its progression systems. There's scarcely any PvP-content and we basically have no real PvE-content in BDO. BDO doesn't even has a real, proper, cohesive community yet it's still popular because the market is starved and there is barely any competition. Like Peon said, there weren't even any real innovations in the genre during the last decade.
