Accuracy, max number of targets, resetting mobs because you attracted them too far from their spawn, or simply bad luck for this mob... I mean, there are plenty of reasons to examine before going on the conspiracy road.
You forgot they are on a slope. A lot of skills have issues if things are one a slope.
