Hi there. In order to make the Inverted Garmoth Heart (I assume that is what you mean) you have to go to the NPC "Hughol" (Blacksmith in Duvencrune). Talk to him while having a Garmoth Heart in your inventory. Make sure to have all quest types enabled. He should then offer you a quest (either called "Valueable Information" or "Inverting the energy"). Take the quest and follow what he tells you. You need to hand in 5 Hesed Crystals (https://bdocodex.com/us/item/9751/) and 3 Concentrated Magical Black Stones (Weapon) to exchange for an item called "Inverted Energy". Then you can heat the Garmoth Heart with the Inverted Energy to get the Inverted Garmoth Heart
